An off-Broadway production "Book of the Dead" Necronomicon Ex-Mortis from George Reinblatt's Evil Dead The Musical. First staged as a workshop in 2003, the musical saw instant success and had an off-Broadway run in 2006. The show was adapted from Sam Raimi's Evil Dead franchise and featured original music.

This stage book, from the personal collection of special effects artist Tom Sullivan, features a slip latex cover showcasing the iconic face of two eyes, a small nose, and a gaping mouth over a cardboard base with woven fabric endpapers hand-sewn into the latex. Bound by Patrick Reese, this book is filled with cardstock pages with colour photocopies of illustrations Sullivan created adhered to both sides. Production intentionally distressed the pages to appear aged. This book features Sullivan's and Reese's signature on the interior. It exhibits buckling to the pages from adhesive. Dimensions: 20.25 cm x 25.5 cm x 5 cm (8" x 10" x 2" )
